Multi-disciplinary Assessments are coordinated by Dr. Farshad Nassiri, Chair of our Medical Advisory Committee (MAC), and require a referral from your doctor.

How ANAC Helps

The referral process for an MDA is available to anyone. As a benefit of ANAC membership, we provide personalized Patient Navigation services to help you:

  • Understand the referral requirements
  • Ensure your submission is complete and followed through
  • Access advocacy and support at every step

Please note that all submissions are triaged, and while timelines may vary, we are here to help you move forward.

MDA Submission Requirements:

  • A note of referral from your Family Doctor or another Specialist identifying the request for an MDA. This should be clearly addressed to Dr. Farshad Nassiri and faxed to: 416-603-5298
  • Complete patient demographics including contact information of Family Doctor
  • Recent MRI imaging must be sent on CD, or provided through Pocket Health (unless imaging was done at University Health Network)
  • All other relevant health and/or medical information that could influence symptoms, assessment, and course of action
