Patient Navigation Services

Multi-disciplinary Assessments (MDAs) are coordinated by Dr. Farshad Nassiri, Chair of ANAC’s Medical Advisory Committee (MAC), and require a referral from your doctor.

How ANAC Helps

The referral process for an MDA is available to anyone. As a benefit of ANAC membership, we provide patient navigation services to help you:

• Understand the referral requirements
• Ensure your submission is complete and followed through
• Receive timely, coordinated care
• Access advocacy and support at every step

Please note that all MDA submissions are triaged.

MDA Submission Requirements

To request an MDA, please ensure the following are submitted:

MDA Referral Form

1. Referral letter from your family doctor or another specialist, clearly addressed to Dr. Farshad Nassiri, accompanied by the MDA Referral Form
2. Complete patient demographics, including contact information for your family doctor
3. Recent MRI imaging (on CD or via Pocket Health, unless completed at University Health Network)
4. Relevant health or medical information that may affect your symptoms, assessment, or care plan
