Multi-disciplinary Assessments are coordinated by Dr. Farshad Nassiri, Chair of our Medical Advisory Committee (MAC), and require a referral from your doctor.

How ANAC Helps

ANAC provides advocacy and support throughout the process, helping ensure your referral is followed and your care is coordinated. Please note that all submissions are triaged, and while timelines may vary, we are here to help you move forward.

MDA Submission Requirements:

  • A note of referral from your Family Doctor or another Specialist identifying the request for an MDA. This should be clearly addressed to Dr. Farshad Nassiri and faxed to: 416-603-5298
  • Complete patient demographics including contact information of Family Doctor
  • Recent MRI imaging must be sent on CD, or provided through Pocket Health (unless imaging was done at University Health Network)
  • All other relevant health and/or medical information that could influence symptoms, assessment, and course of action
